Medical Records

CHI St. Luke's Health-Memorial maintains confidential patient medical records according to the federal and state statutes.

Release of Information

Patient medical records are the property of the hospital. However, patients and/or their representatives may have access to this information with a properly completed and signed release of information form. A valid photo identification is required for all release of information to the patient or their representative. Requests for patient records are handled on a first-come, first-serve basis and require at least 24 hours notice for processing. Depending on the volume and the use of the medical record, there may be a charge based on the state charging guidelines.

You can request that CHI St. Luke's Health-Memorial release medical record information to another party by completing an “Authorization for Release of Information” form. Ensure that all form fields are thoroughly completed. Each section requires a response in order to process your request accurately. You may download the “Authorization for Release of Information" form or you can call or write our office and ask for the form. You may fax, mail, or return the form to the hospital.

Birth Certificates

A birth certificate is completed for all infants born at CHI St. Luke's Health-Memorial. This information is then forwarded to the State of Texas.

Acknowledgement of Paternity (AOP) form may be completed during the infant’s hospitalization or during the first 3 days of the infant’s life if the father’s name is to be included on the birth certificate filed by the hospital. AOP forms can be completed between the hours of 8 a.m. until 3 p.m. Monday through Friday.

Notice to Patients
Destruction of Medical Records

In accordance with the Texas Health and Safety Code, Section 241.103, this notice is given to you regarding the destruction of medical records.

It is the policy of CHI St. Luke's Health-Memorial to authorize the disposal of any medical records on or after the tenth (10th) anniversary of the date on which you, the patient, were last treated at this facility.

If you, the patient, are younger than 18 years of age when you were last treated, CHI St. Luke's Health Memorial may authorize the disposal of medical records relating to you, the patient, on or after the date of your twentieth (20th) birthday, or after the tenth (10th) anniversary of the date on which you were last treated, whichever date is later.

CHI St. Luke's Health-Memorial will not destroy medical records that relate to any matter that is involved in litigation if the hospital knows the litigation has not been fully resolved.
