Polk Education Center

Established in 1994, The Polk Education Center offers specialized nutrition and lifestyle education on a variety of topics including diabetes, heart disease, stroke, weight loss, kidney disease, hypertension and more for both adults and children.

The Polk Center specializes in Diabetes Self Management Training. This type of training is the most successful program for maintaining blood sugar levels within the target zone which can reduce or eliminate the harmful effects of diabetes. The center is certified by the American Diabetes Association.

Located on the first floor of CHI St. Luke's Health-Memorial Lufkin, the center is staffed with trained professionals who develop individualized treatment plans designed specifically with you in mind. From meal planning and medication management to self monitoring and stress management, the Polk Education Center can help you achieve your health objectives.

Education

Professional educators explain treatment options, disease complications, coping skills and barriers to healthy lifestyles. Different learning such as video, literature, example and teach back methods are used in both individual and group settings. Utilizing today’s most current methods, our team of professionals develops a care plan that is as individual as you.

Meal Planning

Making informed food choices is important to all of us, but for people living with diabetes, heart disease or other life threatening conditions, these choices become urgent. Our registered dietitians are here to help you sort through the many food myths today and learn the truth about healthy eating and meal planning. Our dietitians are well trained to help you develop a plan that will contribute to your health and happiness. 

Medication

Our educators teach you how medications work along with proper administration techniques that improve the effect of insulin. When insulin is improperly handled and administered, its blood sugar lowering effect can be reduced.

Self-Monitoring

The Polk Center is a resource for free glucose monitors and can help you access significant savings on the cost of monitoring supplies. Our professionals show you how to calibrate your monitor, check blood sugar and ketone levels and learn the value of regular self-monitoring.

Exercise

Exercise is an important part of any lifestyle program helping to lower blood sugar, blood pressure and cholesterol levels. Polk Center professionals will incorporate exercise goals into your program that are specific to you.

Stress Management

Stress can take its toll on a body, especially those living with diabetes, heart disease or other chronic conditions. Learn how to adopt positive coping skills to deal with stress and rest easy tonight.
